December 2017: Incorrect cutoff when calculating W 
The cutoff procedure for W calculations uses incorrectly Q values for both module MCS and demodularized MCS. Usually the Q for a MCS is higher than its W, which leads to that cutsets in a W calcalution that are below cutoff now surface above cutoff, i.e. are kept in the MCS list when they shouldn´t. However, if Q is smaller than the W for a MCS the MCs might be discarded in the cutoff procedure. For repairable events, which are typically used as initiators in W calculations, this happens if the repair rate is greater than one.
