Feb 2016: Problems calculating 2nd and 3rd order approximation with MUX events 
Minimal cutsets with only one basic event are treated in a special way (as a module) in calculations of the 1st, 2nd and 3rd approximation. this 'modularization' is however switched off if there are basic events defined as mutually exclusive to other basic events in the MCS list. This treatment is correct for the 1st order approximation. However, for the 2nd order approximation and the 3rd order approximation, the current implementation might yield incorrect results. In particular, the 2nd order approximation might be an over-approximation of the exact top value.
